Distinguished Service and Order of Merit Awards are available to any member organization throughout
the year. These awards are sent directly to the organization for presentation at a suitable occasion in the
local community. Please use this form when ordering awards.

Plaques Cost $35.00 each plus applicable taxes, shipping, and handling.
